I agree 100% with the way you have constructed your form. I personally send out a questionnaire to the majority of my prospective clients and have gone back and forth on whether or not to cut this process out and move it all to my contact form.  The only reason I have not done so is that I have found it easier to work the questions in after initial contact. Either way it is a great way to start learning about the client and the project from the start and as you mentioned it certainly works as a filter.
